Care Coordinator?
A Care Coordinator is a professional who is responsible for managing and coordinating the delivery of care services to older people in Australia receiving government-funded Home Care Package (HCP) through My Aged Care. Their role involves assessing the person’s needs, developing a personalised Care Plan, and ensuring the appropriate services (such as personal care, nursing, domestic assistance, or social support) are arranged and delivered effectively.
How We Can Support You
Our team coordinates clinical and non-clinical services, establishes and maintains support and service agreements, and ensures services are delivered according to the Care Plan. We assist in understanding Care Plans or Packages, create plans to overcome barriers to accessing support, plan for unexpected events, and submit requests for plan reassessments.
Our Care Coordinators will act as your trusted adviser and main point of contact. We’ll guide you through understanding the services available to you, whether they are informal, community-based, or government-funded. We’ll work alongside you to address barriers to accessing supports and reduce the complexity of putting services in place, ensuring everything is implemented smoothly.
